How to search for a doctor or facility.

To find an UnitedHealthcare Global provider, visit the WellAway website (link below) and follow these steps:

1. Under the UnitedHealthcare Global icons select one of the following:
   • Physician / Facility Search or
   • Mental / Behavioral Health Search
2. Enter your location (Street, City & State, Zip Code or County)
3. Find Care by Category by selecting the category (People, Places, Service and Treatments, or Care by Condition).
4. Follow the online prompts.
5. Enter your location, then select a provider.

It is best to locate a UnitedHealthcare Global doctor, urgent care center, and emergency room near you before you get sick. Always verify the provider is part of the UnitedHealthcare Global Network before you receive treatment. Always check with the doctor or medical facility directly to confirm that they accept UnitedHealthcare Global before you receive treatment.

All California State University, San Bernardino international students are required to have adequate health insurance coverage for themselves and their dependents if they:

• Hold a current passport and an F-1 or J-1 visa
• Are temporarily residing outside their Home Country while actively engaged in research related activities at the university

The Company maintains its right to investigate student status and attendance records to verify that the eligibility requirements have been met. If and whenever the Company discovers that the eligibility requirements have not been met, its only obligation is a refund of premium, less any claims paid.
